PUR-FORM cost

PUR-FORM publishes a price for most of its wellness menu and for none of its regenerative work. Every stem cell line on the site, from the Muse cell and VSELs infusions to the MiniStem and MaxxStem orthopedic protocols, is listed as Varies against both the member and the non-member rate, so that figure only lands after a consultation. What the clinic does price is the surrounding menu, and those rows are still worth reading because PUR-FORM runs a two tier system where anyone holding a monthly membership pays the lower of the two numbers on every service. Read the published rows as context for your quote, not as your quote.

Membership runs $159 to $529 a month depending on how much treatment is included, and covers the wellness side rather than the cell work. Regenerative and orthobiologic procedures are quoted individually after the consultation. None of it is billed to insurance.

What the price includes

Included

  • The named service and nothing beyond it. PUR-FORM prices service by service rather than as a package, so an orthopedic figure will not carry the IV, hyperbaric or plasmapheresis sessions the clinic tends to pair with it.
  • Member pricing, but only if you hold a membership. Every published row shows two figures and the lower one is gated on a monthly plan that starts at $159.
  • A consultation on the peptide line, which the site lists as a free 60 minute appointment. No other service says the same, so confirm whether an orthopedic or stem cell consultation is chargeable.

Billed separately

  • Everything labelled Varies, which is all of the regenerative work. Stem cell therapy, PRP, MiniStem, MaxxStem, Muse cells, VSELs, exosomes, plasmapheresis, shockwave and hair restoration all carry no published figure.
  • Repeat sessions. The PRP page says most patients have one to three treatments and the stem cell page describes a response measured over weeks to months, but no course price appears anywhere on the site.
  • Imaging. The clinic says it reviews imaging if available and gates candidacy on the evaluation, and it prices neither the evaluation nor any scan.
  • Physical therapy. The clinic highly recommends it after a regenerative procedure and refers out to partner therapists, so that cost sits with a third party.
  • The membership fee itself, if you were quoted the member rate. That is a separate monthly charge on top of the treatment.
  • Supplements. The clinic sells supplement stacks and builds post procedure supplement plans, both priced separately.

Insurance, financing and payment

Insurance

Treat this as self pay. PUR-FORM publishes no insurance policy, no billing page and no HSA or FSA guidance anywhere on its site, and its own footer disclaimer states that nothing on the site has been evaluated or approved by the FDA and describes participation in its protocols as patient funded and voluntary. That is the clinic telling you not to expect a payer to reimburse. If you intend to submit a claim yourself, ask for an itemised receipt with procedure codes before you pay rather than after.

Financing

PUR-FORM partners with Cherry for payment plans and links to it from the navigation on every page. Cherry's PUR-FORM page advertises 0% APR plans subject to eligibility and says applying does not affect your credit, with the loans issued by Cherry's lending partners rather than by the clinic. The exosome and peptide pages both say financing applies to eligible packages without saying which packages qualify, so ask whether your specific procedure is one of them before you assume it is. No deposit policy and no cancellation terms are published. Memberships and gift cards are sold through the clinic's Mindbody account.

How much does PUR-FORM cost?
PUR-FORM does not publish a price for any of its stem cell or regenerative procedures. Stem cell therapy, Muse cells, VSELs, exosomes, PRP and the MiniStem and MaxxStem orthopedic protocols are all listed as Varies against both the member and the non-member rate, so the figure only comes out of a consultation. The rest of the menu is priced. EBO2 ozone therapy is $2,300 for a non-member and $1,955 for a member, a Niagen IV is $499 or $449, a hyperbaric session runs $199 to $315, an entry level IV drip is $209 or $169, and a basic vitamin shot is $35 or $25. Memberships are $159, $269 or $529 a month.
Why is there no price for stem cell therapy?
Because the clinic builds the procedure per patient and the components stack. It offers both autologous cells, harvested from your own bone marrow or fat, and allogenic donor cells, and its orthopedic ladder runs PRP, then MiniStem which adds an adipose harvest, then MaxxStem which adds bone marrow on top of that. Each step adds a harvest and lab processing, which is the usual reason a clinic quotes privately rather than listing. What the site never says is how much any one of those steps adds, so the only way to compare the three is to ask for all three quoted on the same document.
What is the difference between the member and the non-member price?
Every published row carries two figures and the lower one requires a monthly membership. The tiers are $159, $269 and $529 a month, bundling 5, 10 or 30 wellness treatments plus member pricing across the menu, and the clinic claims monthly savings of over $335, $845 and $2,045 on the included services. Whether that pays back on a one off regenerative procedure is impossible to work out from the site, because the regenerative rows show Varies at both tiers with no member discount stated. Ask what the membership actually takes off a stem cell quote before you sign up for one to get it.
What should I ask for in writing?
Ask for the recommended protocol named explicitly, whether that is PRP, MiniStem, MaxxStem, Muse cells or VSELs, with a total against it. Ask whether the figure is per session or per course, and how many sessions the surgeon expects. Ask whether both sides or multiple joints are included or billed separately. Ask what the harvest, the evaluation and any imaging add. And ask whether Cherry financing is available on that specific procedure, since the clinic only says financing covers eligible packages without listing them.
