How it is scored
Ten sections, each with six statements running from no limitation to complete limitation, scored zero to five. The total out of fifty is expressed as a percentage. Fifty percent means back pain is limiting half of what the questionnaire measures.
Like the knee tool, this is written for this page rather than reproducing a licensed instrument. It follows the structure of a disability index and covers the same areas of daily life, so it tracks the same thing and moves the same way, but a clinician wanting a formal Oswestry score will administer the real questionnaire.
What each band usually means
0 to 20 percent, minimal. Most people here are managed with activity advice, exercise and time. Bed rest is specifically not the answer, and the evidence on that is not close.
20 to 40 percent, moderate. Sitting, lifting and standing are affected. Conservative care is still first line, and physiotherapy at this stage does more than most people expect.
40 to 60 percent, severe. Pain is the main problem in daily life. This is where imaging and a specialist opinion are normally arranged, and where the more invasive options start being discussed.
Above 60 percent. Pain intrudes on every area of life. This needs a clinician, promptly, and not an internet questionnaire.
Signs that need attention now
Some back pain symptoms are not a matter of scoring. Loss of bladder or bowel control, numbness around the groin or inner thighs, progressive weakness in a leg, or severe pain following a significant injury all need urgent medical assessment rather than a self-assessment score. Back pain and regenerative treatment
Clinics offer cell therapy for degenerative disc disease and facet joint pain, among other back diagnoses. The evidence base is thinner than for knee osteoarthritis and the anatomy is harder, since a disc is a poorly vascularised structure that is difficult to deliver anything into and difficult to assess afterwards. Ask any clinic which specific diagnosis it is treating, what it expects to change, and how that change will be measured. A dated score before and after is one reasonable answer to the last part.

Is this the Oswestry Disability Index?
No. It follows the same ten-section structure and the same zero to five scoring, with statements written for this page rather than reproduced from the published instrument. It tracks the same thing and moves in the same way. A clinician needing a formal ODI score will use the validated version.
What is a normal back pain disability score?
Below 20 percent is generally treated as minimal disability, which is where most people with ordinary episodic back pain sit. Anything above 40 percent means pain is dominating daily function and is worth a specialist assessment.
When should I see a doctor urgently?
Loss of bladder or bowel control, numbness around the groin or inner thighs, progressive leg weakness, or severe pain after a significant injury all need same-day medical assessment. So does back pain with fever, or in anyone with a history of cancer. These are not questionnaire situations.
Can stem cell therapy help back pain?
Some clinics offer it for degenerative disc disease and facet joint pain. The evidence is less developed than for knee osteoarthritis, and delivering anything into a disc and then measuring what happened is genuinely difficult. Ask for the specific diagnosis being treated and what published results exist for it.
