Cord blood, the blood left in the umbilical cord and placenta after a baby is born, contains blood stem cells (also called hematopoietic stem cells) capable of producing every type of blood and immune cell in the body. It's one of three sources of these cells, alongside bone marrow and peripheral blood, and it's been used in transplant medicine since the first cord blood transplant in 1988. Since then, more than 40,000 transplants have used cord blood as the source.

What cord blood stem cells actually treat

Blood stem cell transplantation, whether the cells come from cord blood, bone marrow, or peripheral blood, can treat and in some cases cure diseases of the blood and immune system. This includes certain blood cancers, red blood cell disorders, bone marrow failure diseases, and some immunodeficiencies. Which source gets used depends on the patient's age, their specific disease, and whether a suitable donor match is available.

It's worth being precise here: cord blood stem cells are established, evidence-backed treatment for a defined set of blood and immune conditions, through a well-studied transplant process. That's a different thing from the broader, less-established uses sometimes marketed by clinics for unrelated conditions, more on that below.

Cord blood vs. bone marrow vs. peripheral blood

Each source has real tradeoffs:

Advantages of cord blood:

  • Lower risk of transmitting blood-borne infections

  • Lower risk of graft-versus-host disease, a potentially serious immune reaction where donor cells attack the recipient's tissue

  • Can offer a match for patients who can't find one in bone marrow donor registries

Disadvantages of cord blood:

  • Fewer stem cells per sample than bone marrow or peripheral blood, often requiring two cord units for a single adult transplant

  • Slower immune system recovery after transplant, raising infection risk during that window

  • No ability to collect additional sample later, unlike bone marrow or peripheral blood, which can be re-collected if needed

Public donation vs. private banking

This is the decision most expecting parents actually face, and it's worth understanding both sides clearly.

Public banking means donating cord blood for anyone who needs a match, along with research use. The family gives up rights to the sample, but public banks make donations broadly available, they've released roughly 30 times more units for treatment than private banks have, despite private banks holding far more total units in storage. Medical bodies including the American Academy of Pediatrics generally recommend public donation over private banking for most families.

Private banking stores cord blood exclusively for the family's own future use, for an initial collection fee plus ongoing storage costs. The odds of a person ever needing their own banked cord blood are low, estimated at less than 1 in 20,000. Private banking is more strongly recommended in specific cases: when a full sibling already has a disease treatable by stem cell transplant, since some private banks will even store for free in that situation.

One important caveat: if a child has an inherited genetic blood or immune disorder, their own cord blood carries the same genetic mutation and generally can't be used to treat that same condition without additional gene therapy, an area still being researched.

What to check before choosing a bank

Whichever direction a family leans, accreditation matters. Look for banks accredited by the Foundation for the Accreditation of Cellular Therapies (FACT) or AABB, organizations that set recognized standards for cord blood banking and cell transplantation.

A caution worth taking seriously

Cord blood's established, evidence-backed uses are specifically for blood and immune system diseases treated through transplantation. Some clinics market cord blood, or related cells from placenta or amniotic tissue, for a much broader range of conditions unsupported by current science. This pattern, promising outcomes ahead of the actual evidence, is common enough among stem cell clinics generally that it's worth treating any such claim with real scrutiny, and confirming with your own physician what's actually established for your specific situation.
